That's right - no one else knew Archer had the surgery not, including his wife, his boss (the director of the FBI), or the warden of the prison he's going to be held in. Once Castor has Archer's face, he kills Archer's handler, the surgeon, and a lone FBI agent, to make sure no one knows what happened.Sure it would be a Heroic Sacrifice, but that's preferable to him allowing a terrorist to steal the identity of a high-ranking FBI agent. The surgeon is held at gunpoint and forced to perform the procedure, but surely he knew he would be killed when he was done, and in the meantime he could have "slipped" his hand and killed Caster. This means when he wakes up and realizes what they've done, he can easily call his crew, have them abduct the surgeon who did the procedure, and force him to give him Archer's face. After Archer takes Caster's face, his own face is left in the hospital, and Caster is left in his hospital bed with no security and no anesthetic keeping him comatose.
